At its core, pathology is the study of disease. Diseases occur for many reasons. But some, such as cystic fibrosis and Parkinson’s Disease, occur because of alterations that prevent cells from functioning normally.

Understanding of signals and symptoms of alterations in cellular processes is a critical step in diagnosis and treatment of many diseases. For the Advanced Practice Registered Nurse (APRN), this understanding can also help educate patients and guide them through their treatment plans.

For this Discussion, you examine a case study and explain the disease that is suggested. You examine the symptoms reported and explain the cells that are involved and potential alterations and impacts.

By Day 1 of this week, you will be assigned to a specific scenario for this Discussion. Please see the “Course Announcements” section of the classroom for your assignment from your Instructor.


Post an explanation of the disease highlighted in the scenario you were provided. Include the following in your explanation:

· The role genetics plays in the disease.

· Why the patient is presenting with the specific symptoms described.

· The physiologic response to the stimulus presented in the scenario and why you think this response occurred.

· The cells that are involved in this process.

· How another characteristic (e.g., gender, genetics) would change your response.

Read a selection of your colleagues’ responses.

Week 1 Discussion

By Day 1 of Week 1, choose a case study scenario for this term.

Scenario 1:  Patient is a 22-year-old male who presents to the clinic for allergy immunotherapy “allergy shots”.  The patient is administered the medication the injection and waits 30 minutes to leave the center.  As the patient is leaving, he begins to complain of having difficulty swallowing, nausea, and feeling weak.  His vitals are 100/76, HR 120, Resp 24, O2 sat 95%.  The patient is now seating in a tripod position on the table.

Scenario 2:  Patient is an 18-year-old female with a diagnosis of cystic fibrosis who presents with complaints of increased productive cough and fatigue in the last few days.  The patient denies a fever but reports that her appetite hasn’t been good because she is so tired.  Patient vital signs:  BP 110/70, P 110, Res 28 labored, temp 98.4. Physical exam:  Lung sounds Diminished breath sounds bilateral lower lobes with crackles in upper lobes. 

Scenario 3:  Patient is a 48-year-old male with complaints of stiffness in his fingers that migrates to his upper arm joints and increased fatigue over the last few months.  The patient works in a bank and types frequently.  He commented that he thought he could have carpal tunnel syndrome, but the fatigue was beginning to worry him.  The patient reports a family history of rheumatoid arthritis.  He is currently taking Motrin for pain which partially relieves the symptoms.  He rates his pain a 5/10 at present, aching in both hands.  The patient’s vital signs are 120/80, Pulse 78, Res 16, Temp 100.1 F.  He denies any recent weight changes.  Physical exam reveals:  Edema to 4th and 5th PIP joints bilaterally with tenderness in the same joints.  His grip strength is decreased bilaterally. No edema of knees or ankles noted. 

Scenario 4:  Patient is a 42-year-old male who works as a carpenter with complaints of jerky involuntary movements which have become more pronounced in the last few months.  He first noticed that he was unable to grip his hammer at work.  He felt that his symptoms were related to caffeine intake in the beginning but have become worse and is now impacting his ability to perform his job duties.  He has been having some increased stress at home due to financial issues due to a possible work layoff.  His wife recommended he be seen for evaluation.  His paternal grandfather was diagnosed with Huntington’s disease at 48 years old.

Scenario 5:  Patient is a 38-year-old female who presents to the clinic with complaints of a wound to her right leg after a recent fall 5 days ago.  She reports that the area appeared to be healing and has now started getting red and has some dark colored drainage.  She denies any odor to the site.  She denies fever.  She has been cleaning the area with a wound spray and applying antibiotic ointment with a Band-Aid.  She reports that she has had some pain to the site 4/10 sharp at times.
